North and West Remote Health (NWRH) is seeking a dedicated Care Partner to join our Community Services team in Mount Isa. About the Role

As a Care Partner, you will contribute to the development, implementation, monitoring, and review of individualised care plans that support older Australians to remain independent and connected to their communities.

Working within a multidisciplinary team, you will provide care coordination, case management, and service navigation while ensuring services align with best practice, funding requirements, and client goals.

Key Responsibilities

  • Conduct comprehensive assessments of clients and their home environments
  • Develop, implement, monitor, and review person-centred care plans
  • Coordinate and evaluate supports delivered under programs including Support at Home, CHSP, and other community-based services
  • Support clients and their families to make informed decisions about their care
  • Assist clients to navigate aged care systems and access available supports
  • Monitor client outcomes and adjust supports as needs change
  • Liaise with internal and external health professionals to coordinate care
  • Facilitate referrals to clinical and specialist services when required
  • Maintain accurate documentation and reporting in line with funding and quality requirements
  • Build strong relationships with clients, carers, families, and community stakeholders
  • Participate in outreach service delivery across regional and remote communities

About You

You are passionate about supporting older Australians and understand the importance of delivering respectful, person-centred care.

You are highly organised, communicate effectively, and enjoy building strong relationships with people from diverse backgrounds. You thrive working independently while also contributing to a collaborative multidisciplinary team.

Essential Requirements

  • Demonstrated experience in community aged care, case management, care coordination, or related support services
  • Knowledge of community aged care programs and funding models
  • Understanding of person-centred and community-based service delivery
  • Strong interpersonal, communication, and relationship-building skills
  • Excellent written communication and administrative skills
  • Intermediate computer skills, including Microsoft Office applications
  • Ability to work autonomously and within a multidisciplinary team environment
  • Awareness of local health issues and experience working with people from diverse cultural backgrounds
  • Current unrestricted C Class Driver Licence
  • Current Blue Card and NDIS Worker Screening Check
  • Australian or New Zealand citizenship, permanent residency, or unrestricted working rights in Australia

Highly Regarded

  • Diploma or Degree in Community Services, Aged Care, Health, or a related discipline
  • Experience working within Support at Home, Home Care Packages, CHSP, or similar aged care programs
  • Experience coordinating services across rural and remote communities
  • Strong digital literacy and confidence using smartphones, tablets, laptops, and electronic client management systems
  • Understanding of wellness, reablement, cultural safety, and trauma-informed care principles

Travel Requirements

This position is based in Mount Isa and includes regular outreach travel throughout the NWRH catchment area.

Travel may involve road travel and occasional flights in small aircraft, including overnight stays of up to four nights approximately once per month.
